The CPG reinforces: CLBP demands a **multimodal, active-treatment approach.** It's time to fully integrate active exercise, skilled manual therapy, and effective education.

Pillar 1: Exercise - The Foundation

For CLBP, structured, progressive activity is non-negotiable. It's where lasting change begins.

PTs **SHOULD** Use:

  • Trunk muscle strengthening & endurance
  • Multimodal exercise interventions
  • Specific trunk muscle activation
  • Aerobic, aquatic, & general exercise
  • For older adults: General exercise to reduce pain & disability

Pillar 2: Manual Therapy - The Powerful Adjunct

Use MT to open a window of opportunity for active engagement, not as a standalone fix.

CPG Recommendations:

  • PTs **SHOULD** use thrust/nonthrust joint mobilization.
  • PTs **MAY** use soft tissue mobilization or dry needling *with* active treatments.

Pillar 3: Education - Empowering the Patient

Education alone isn't enough; it must fuel action and self-efficacy.

CPG Recommendations:

  • PTs **SHOULD** deliver Pain Neuroscience Education (PNE) *alongside* other PT interventions.
  • PTs **SHOULD** use active treatments (yoga, strength, etc.) *instead of* stand-alone educational interventions.

Your Clinical Mandate: Actionable Insights

  • LEAD WITH ACTIVE CARE: Make progressive exercise and movement control the core of your program.

  • MOBILIZE FOR OPPORTUNITY: Use manual therapy to reduce pain and facilitate better movement, directly integrating it into active interventions.

  • CONTEXTUALIZE PAIN: Pair PNE with exercise to build self-efficacy and demonstrate resilience, shifting narratives from vulnerability to capacity.
