We’ve all been taught that Heavy-Slow Resistance (HSR) is the "Gold Standard" for patellar tendinopathy. And it is… if the patient can handle it.
But what happens when the tendon is so irritable that even a moderate load feels like a lightning bolt to the infrapatellar pole?
A new RCT presented at the 2025 Scandinavian Sports Medicine Congress just dropped some major clinical utility:
Low-Load Blood Flow Restriction (LL-BFR) vs. Heavy-Slow Resistance.
The results? Equivalence.
✅ Pain & Function (VISA-P): Comparable.
✅ Quad Strength: Comparable.
✅ Tendon Structure: No difference.
The Clinical Takeaway: You don’t have to sacrifice strength gains just because a patient is in a high-pain state. LL-BFR allows you to keep the "engine" (the quads) running while the "chassis" (the tendon) settles down.
New Evidence for Patellar Tendinopathy Rehab

When your patients can't tolerate the high compressive/tensile loads of traditional Heavy-Slow Resistance (HSR) training due to high irritability, Low-Load BFR is a evidence-backed, viable alternative. You can maintain strength and promote recovery without the "heavy" pain.
